* You must have a Waybill and envelope filled out completely. Please Make sure to fill out the last time feed and water.

* All animal(s) should be booked by Sunday at 10pm for the following Monday.

* All applicable USDA paperwork filled out completely.  

* Health Certificate(s) by a veterinarian is required for all animals to be transported.  There must be identification numbers for each animal shipped on the health certificate.  
 We need 2 copies of the health certificate.  One is to follow the animal(s) and one for our file.

*  Health Certificate(s) are only recognized for 10 days including days of travel from the date it was issued.

*  All animals must be free from parasites( fleas, ticks, lice, and worms.)

*  All animals must be current on vaccinations.  Animals 80 days and over must have a rabies vaccination.

*  Collar All animal(s) with destination, city and state, ID#, Breed, sex, USDA #, your last name, microchip # if chipped, and the date of birth or age. Microchip is not required for shipping.  We require this for transporting animals.

*  Label(s) for transport   that matches info on the collar.  You can get these at Wal-mart or Dollar General in the office supplies dept.)  Required for transporting animals.  Each animal must have their own label.

*  Example of collar and label:

                                                ID#        Breed        Sex       Your last name

Please paper clip your waybill, envelope, one copy of the health certificate, and USDA (if applicable) together in this order.
Any other paperwork needs to be in the envelope with additional copy of the health certificate.
